I have it set to fullview automatically, because I always fullview anyways and it saves the extra click
If it's a truly enormous piece, then usually I just go back before it finishes loading...people do really need to learn to resize (although I understand if it's another upload of the same piece, only bigger, for people who want to see tiny details, but then I would know from the name of the picture that it's just a detail upload that I'm not interested in).
I had a mixed answer but basically it is Yes, Always. But really I only click on a thumb if it is something that looks interesting. But if I click, then I always full view.
I fullview if the medium-size view seems to warrant a full view. And that does not include previews showing something else than the full view, with FULL VIEW PLEASE stamped across it.
I hate it too, but maybe not for the same reason: to me it looks like an attempt to force me to bump their full-views counter without me being sure that it could be worth it — akin to mensogous advertising or selling damaged goods.

Though I must say I'm sad when I fullview and it's the same size as the thumbnail. If only the internet was safe, could artists show their images..
